Prompting
A reminder to take your medication at the right time. Your worker checks that you have taken it as prescribed.

Medication Support is help with taking your prescribed medication as part of an agreed care arrangement. The level of support you receive depends on your assessed needs.
This is not a standalone service. Medication Support is part of your wider Home Care arrangement, delivered by a worker who is trained and verified for the level of support your care plan requires.

A reminder to take your medication at the right time. Your worker checks that you have taken it as prescribed.
Help with opening containers, measuring doses or preparing medication for you to take.
Direct administration of medication where your care plan requires it and the worker is trained and authorised to provide it.
The level provided to you is set during your care assessment and recorded in your care plan. Workers are matched to your needs based on their verified training and competence. We do not provide medication support beyond what the care plan authorises or what the worker's competence allows.
Medication Support is governed by your agreed care plan, with records maintained for every visit.
Your worker follows specific instructions for each medication, including timing, dosage and method.
If there is a concern about your medication or a change in your prescription, we review and update the arrangement.
